In its breadth and diversity, this 4th edition contains wide ranging and systemically arranged information on both digestive and extradigestive carcinoids (ECCs/NECs) and their relevant endocrinocarcinomas as well as pancreatic islet cell neoplasms. A few minor changes are made in the fourth Edition with additional 4 articles that are of a considerable importance. Two of them are included in the last Chapter of "Some problems in carcinoid research", describing some evolutional proposals based on the author's recent concept. The author points out the inadequacy of the term "carcinoid" and emphasizes advisable warnings that the term was proposed on a malignant submucosal nodule of the small intesttine which was misjudged as a benign lesion. The term “carcinoid” is a misnomer and should not have been in the history. Also very early carcinoid formation is clearly demonstrated in a form of budding from the basal portion of a gastric gland, proving that a carcinoid is malignant at its origin.   It is hoped that young physicians and researchers alike will benefit from these detailed studies when encountering such cases in the future.
